Title : Outpatient and C-Change Intensive Care Coordinator (ICC)
Classification : Regular Full Time (1.0 FTE), Non-exempt (Hourly)
Location : Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely)
Regular Work Schedule : Full time, M-F
Compensation:
- $34.62 per hour
The Mental Health Care Coordinator, formally known as the Intensive Care Coordinator (ICC) is responsible for the assessment, development, coordination, implementation, and oversight of intensive behavioral health, community-based care plans for moderate to high-acuity clients with complex behavioral health, medical, developmental, and/or psychosocial needs. The ICC identifies and brings together family, natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, school, juvenile probation, etc.) to form a Child & Family Team (CFT) which collectively works together to address goals and ensure successful and enduring change.
The ICC exercises clinical judgment and discretion in collaboratively evaluating client needs and strengths, determining appropriate interventions, coordinating interdisciplinary treatment services, managing crises, resource linkage, behavioral skill building and developing individualized care strategies guided by the Integrated Core Practice Model. The ICC coordinates and facilitates regular CFT meetings among teams, and monitors and contributes to progress toward a youth's goals.
The work is varied in character, requiring interpretation of clinical information, application of professional standards, and clinically informed decision-making. ICCs provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.). This position primarily serves youth and young adults in the C-Change program.
About the C-Change Program
The C-Change Program, supports the transformation of the lives of youth who are at risk or have been identified as sexually exploited minors by focusing on supporting stabilization, reducing mental health symptoms, self efficacy and self determination, community building, and connecting youth with other resources. Our specialized approach provides mental health screening, advocacy, clinical case management, and psychotherapy to at-risk youth. C-Change provides services to youth who are up to 21 years old and reside throughout the Bay Area.
Responsibilities & Competencies:
- Assessment and Care Planning - Conduct comprehensive biopsychosocial, behavioral health, risk, and functional assessments for clients with complex and high-acuity needs. Support the development, implementation, and monitoring of individualized care plans using evidence-based and trauma-informed practices. Evaluate clinical progress and modify interventions based on ongoing assessment and professional judgment.
- Facilitate Child & Family Team (CFT) Meetings - Regularly schedule, organize and facilitate CFT Meetings with youth/families and their community stakeholders. Facilitation training will be provided.
- Team-Based Care - Enthusiastically able to work collaboratively as part of a multidisciplinary treatment team to support youth and families with developing and making progress toward their goals.
- Clinical Care - Manage a caseload of up to 19 clients and ensure 50% (20 hrs / week) are dedicated to providing behavioral health services to youth and families; Uphold ethical and legal responsibilities, complete high quality documentation, link youth to resources, support skill building, assess needs, strengths & provide crisis intervention and de-escalation. Represents our mission, philosophy, and upholds ethical and legal responsibilities as a mandated reporter
- Crisis Intervention & Clinical Decision Making - Independently and collaboratively assess and respond to behavioral health crises, safety concerns, and urgent client situations. Exercise professional judgment in determining appropriate interventions, escalation procedures, mandated reporting obligations, and emergency referrals.
- Cultural Humility - Awareness of the impacts of power, privilege, race and SOGIE on self, clients & others.
- Efficiency and Time Management - Ability to be well-organized, manage time/schedule effectively, prioritize and complete timely documentation.

- Relationship-Centered - Understands the value of building healthy relationships to help youth navigate complex systems and how trauma can impact youth and team dynamics; Open to Restorative Justice Practices.
- Commitment to Ongoing Learning - Ability to use supervision effectively; Ability to be self-reflective and accountable to growth areas; passionate about learning and participating in ongoing training provided by WCC. Participate in individual and group supervision, program meetings and trainings consistently.
- Progress Notes and Clinical Documentation - Knowledge of or ability to learn Medi-Cal reimbursement procedures and billing; This position requires the completion of daily progress notes and other clinical documentation which complies with policies and procedures for meeting EPSDT Medi-Cal and other billing requirements as set by WCC.
Qualifications:
- At least 2 years of professional experience working directly with children and/or families from a mental health framework in behavioral health, care coordination, intensive case management, social services, healthcare coordination, or related clinical services.
- Group, training or treatment team facilitation experience and skills are required.
- Bachelor's or Master's degree in Psychology, Social Work, Human Services, Counseling, or a related field required. Candidates with significant relevant professional experience in mental health or behavioral health services may be considered in lieu of a degree.
- Demonstrated experience working with adolescents/minors with dual diagnosis, high mental health needs, and/or have experienced child sexual exploitation (CSE).
- Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required.
- Ability to apply professional judgment in ambiguous and evolving situations.
- Strong crisis assessment and intervention skills.
- Passionate about working with foster youth, families & young adults who have complex traumatic histories.
