As part of a cohesive and motivated team, you will work closely with other healthcare professionals to ensure comprehensive care. The role is structured around a Monday to Friday schedule, allowing for a balanced work-life dynamic while providing essential services to our community.Key ResponsibilitiesConduct thorough evaluations to assess patients' physical conditions, integrating relevant medical history and conducting specific assessments to establish a baseline and identify treatment needs.Collaborate with patients and their families to explain evaluation findings, set realistic treatment goals, and develop a comprehensive treatment plan tailored to the patient's unique needs.Implement treatment protocols effectively based on evaluation outcomes, ensuring modifications are made as the patient's condition evolves or if additional medical input is required.Maintain accurate and timely documentation of all patient interactions including initial evaluations, daily notes, progress reports, and discharge summaries to ensure continuity of care.Engage with multidisciplinary teams as needed, fostering communication and collaboration to enhance patient care and address any barriers to access.Participate in community outreach and marketing initiatives to promote the organization's services and connect with broader patient populations.Take responsibility for your professional development by pursuing continuing education opportunities and serving as a mentor to students in the field of physical therapy.Commit to delivering superior customer service, adapting treatment approaches to accommodate the needs of diverse patients, ensuring their experience with us is both positive and supportive.Patient Population / Client FocusThe ideal candidate will work with a diverse group of patients ranging from post-operative recovery to chronic condition management, providing them with personalized care that enhances their quality of life. Our clients value flexibility, compassion, and expertise, and as their physical therapist, you will play a critical role in their rehabilitation journey.QualificationsA Doctorate of Physical Therapy (DPT) from an accredited program.Active state license to practice as a Doctor of Physical Therapy.A minimum of two years of relevant experience in physical therapy, ideally with experience in home health settings.Current CPR Certification.Preferred ExperienceWhile two years of experience is required, candidates with experience in orthopedic settings or specialized therapy techniques are highly preferred. Familiarity with health care documentation systems and a penchant for innovative treatment methodologies will also be advantageous in this role.Compensation and BenefitsWe offer a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
