Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify clients’ mental health concerns and strengths, focusing on their needs related to hospice care.

• Deliver individual, family, and group therapy that is tailored to the unique emotional landscapes of clients approaching end-of-life transitions.

• Utilize evidence-based therapeutic modalities including cognitive-behavioral therapy, solution-focused therapy, and trauma-focused interventions to address anxiety, grief, and depression.

• Develop and implement individualized treatment plans that incorporate achievable goals and interventions, revising them as necessary to adapt to clients’ evolving circumstances.

• Collaborate closely with a multidisciplinary team, including medical professionals, psychologists, and chaplains, to coordinate holistic patient care.

• Provide crisis intervention and conduct risk assessments to ensure the safety and well-being of clients in distress.

• Facilitate group therapy sessions and psychoeducational classes to support clients and families in processing their experiences.

Qualifications\n
