Salary Range: $73,924.50 - $92,410.50 per year

Location: Bronx or Harlem, NY

Position Summary
As a Registered Dietitian within a long-term Medicaid/Medicare program serving seniors, you will support participants who require nursing home-level care while enabling them to remain safely in their homes. You will provide comprehensive nutritional assessments, develop individualized care plans, and deliver education and counseling to promote health and quality of life. Working as part of a dedicated interdisciplinary team, you will help ensure coordinated, patient-centered care.

Responsibilities

• Shift details: Monday - Friday, 8:30am - 5:00pm
• Hours per week: 40

? Key Duties:

• Assess nutritional status of participants and develop individualized nutrition care plans
• Provide nutrition counseling and education to participants and caregivers using appropriate teaching materials
• Monitor participant progress and revise nutrition plans to ensure optimal outcomes
• Document all care activities and maintain compliance with organizational policies and regulatory standards
• Collaborate with interdisciplinary teams and oversee dietary needs in coordination with food services staff

Requirements

? Education

• Required: Bachelor's Degree in Food and Nutrition or related field

? Licenses & Certifications

• Required: Registered Dietitian (RD) in the state of New York

? Skills:

• Strong knowledge of clinical nutrition and geriatric dietary needs
• Excellent communication and counseling skills
• Ability to assess, plan, and implement individualized nutrition care strategies
• Strong organizational skills and attention to detail
