Community Physicians
Community Physicians is seeking a Board-certified NP or PA to join our outpatient geriatric care team. You’ll co-manage a defined panel alongside a lead physician, collaborating with a dedicated team to deliver prevention, chronic disease management, and goal-oriented care in a clinic setting.
