The Case Manager provides participant-centered support to individuals working toward permanent, stable housing. Using a Housing First and participant-led approach, this role partners with participants to identify their housing goals and determine the steps needed to achieve them.
Case Managers provide strengths-based guidance, advocacy, and resource navigation while helping participants access services, build life skills, and overcome barriers within housing, medical, legal, and social service systems.
Many participants served by the program are experiencing homelessness and may face additional challenges such as disability, trauma, legal barriers, or language barriers. Spanish language skills are highly valued, as several participants benefit from additional support navigating housing, legal, and medical systems.
This role helps create an environment of dignity, empowerment, and opportunity, supporting participants as they work toward stability and independence within the community.
Participant Advocacy
- Advocate for participants within housing, medical, legal, and social service systems
- Support participants in navigating housing applications, documentation, and service coordination
- Assist participants in communicating with service providers and addressing barriers to housing stability
Goal Setting & Participant Support
- Collaborate with participants to identify strengths, establish housing goals, and create action plans
- Provide encouragement and mentorship that supports participant empowerment and independence
- Offer ongoing guidance during transitions and periods of instability
Resource Navigation
- Connect participants with community resources including housing, employment, education, transportation, healthcare, and other essential services
- Provide referrals for resources such as clothing assistance, food support, treatment services, and overdose prevention education
Life Skills Development
- Support participants in building practical life skills such as budgeting, appointment management, and household organization
- Encourage self-advocacy and independent navigation of service systems
Community Collaboration
- Maintain communication with individuals involved in the participant’s housing journey, including family members, housing navigators, and service providers
- Support participants in building both formal and informal support networks within the community
Participant Support Services
- Transport participants to appointments or housing-related meetings when necessary
- Assist participants in obtaining critical documents such as birth certificates and Social Security cards
- Accompany participants on housing tours and assist with room inspections Documentation & Compliance
- Maintain timely and accurate documentation of participant interactions, goals, and program spending
- Ensure records comply with confidentiality standards related to housing and medical information

Participant-Centered Approach: Experience working with marginalized communities, including people experiencing homelessness, communities of color, and individuals with disabilities. A strong commitment to treating all participants with dignity and respect.
Organization & Time Management: Ability to manage multiple cases and responsibilities while maintaining accurate documentation and consistent follow-up with participants.
Flexibility & Resilience: Comfort working in a dynamic environment and supporting participants through complex challenges while remaining calm and solution-focused.
Equity Focus: Commitment to recognizing and addressing systemic barriers that impact access to housing, services, and opportunities.
Case Management Experience: Experience navigating social service systems and supporting individuals with complex needs through coordinated services and advocacy.
Required:
- Successfully complete a criminal background check
- At least 2 years of experience in social services, homeless services, or a related field.
- Knowledge of trauma-informed care, harm reduction, and Housing First principles.
- Strong interpersonal, crisis intervention, and problem-solving skills.
- Valid Drivers License

Preferred
- Spanish language proficiency
- Experience in housing services, case management, or social services
- Familiarity with local housing systems and coordinated entry processes.
Typical schedule: Monday–Friday, 9:00 AM – 5:00 PM
Participant office hours: 11:00 AM – 4:00 PM , during which staff are expected to be on-site and available to participants.
