Certified Community Health WorkerWe are seeking a compassionate and motivated Certified Community Health Worker (CCHW) to join a growing healthcare team serving communities across Florida.Requirements:Active Florida Certified Community Health Worker (CCHW) certification or currently enrolled in a certification program.Candidates with an active Community Health Worker certification from another state are also encouraged to apply.Must reside in Florida.Strong communication, organization, and problem-solving skills.Ability to build rapport with diverse populations while providing compassionate support.Responsibilities:Connect members with community resources and healthcare services.Conduct outreach and provide education on available programs.Maintain accurate documentation and case notes.Collaborate with care teams to support member needs and improve health outcomes.Assist members in overcoming barriers to accessing care and social services.This is a fully remote opportunity offering $23/hour for candidates passionate about making a difference in their communities.
