High Needs Case Manager Children Services

Spectrum Healthcare

The Case Manager works under professional supervision to coordinate care, support treatment planning, and provide rehabilitation and care management services. This role plays a key part in helping individuals navigate behavioral and physical healthcare systems while ensuring services are delivered in alignment with clinical standards and regulatory requirements. Minimum Qualifications Candidates must meet Behavioral Health Technician (BHT) requirements through one of the following: High school diploma or GED and at least 2 years of behavioral health experience (education and experience may be combined), OR Bachelor's degree (any field) and at least 1 year of behavioral health experience, OR Bachelor's degree in a human services-related field, OR Completion of a Behavioral Health Technician training program, OR Master's degree in a human services-related field (not licensed) Preferred: At least 1 year of behavioral health experience For High Needs Care Managers: 2 years of care management experience Bachelor's degree in a human services-related field or equivalent combination Additional Requirements: Must complete required training within 6 months of hire if not already completed Valid driver's license with a clean driving record Key Responsibilities Coordinate care and implement treatment plans under supervision Complete assessments, reassessments, and documentation in compliance with guidelines Provide supportive counseling and maintain accurate clinical records (DIAP format) Collaborate with internal teams, providers, and community agencies Ensure compliance with federal, state, and payer regulations Maintain productivity standards and participate in team meetings and reviews Deliver culturally competent services to diverse populations Support care coordination across behavioral and physical health services Maintain professionalism, confidentiality, and ethical standards High Needs Care Manager (Additional Duties) Provide intensive case management for high-needs populations, including SMI individuals Maintain weekly contact with clients (home, community, or clinical settings) Assist with transitions in care (hospitalization, incarceration, residential placement) Coordinate services with agencies such as DCS, probation, schools, and DDD Facilitate Child & Family Team (CFT) and Adult Recovery Team (ART) meetings Complete referrals, authorizations, and required documentation Collaborate with clinical teams to ensure timely and accurate service planning Skills and Competencies Strong care coordination and case management skills Knowledge of behavioral and physical health conditions and interventions Understanding of DSM-5 diagnostic criteria Ability to conduct crisis assessments and interventions Strong documentation #J-18808-Ljbffr Spectrum Healthcare
