Physical Therapist (PT)
A healthcare organization is seeking a dedicated Physical Therapist (PT) to provide rehabilitative care and help patients improve mobility, function, and overall quality of life. This role is ideal for a compassionate professional who is committed to delivering patient-centered care in a collaborative healthcare environment.
Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical findings and rehabilitation goals
- Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
- Monitor patient progress and modify treatment plans as needed
- Establish realistic and achievable rehabilitation goals
- Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely
- Educate patients and family members regarding treatment plans, exercises, and recovery strategies
- Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care
- Supervise and support Physical Therapist Assistants as appropriate
- Maintain compliance with professional standards, regulatory requirements, and patient confidentiality guidelines
- Promote a safe, positive, and patient-focused care environment
- Perform additional duties as assigned
Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license or eligibility for licensure in the state of practice
- Strong clinical assessment and treatment planning skills
- Experience with rehabilitation documentation and patient progress tracking
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Experience in rehabilitation, long-term care, skilled nursing, or geriatric settings preferred
