Salary range: $158,000.00 - $185,000.00 Salary/year

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) . The selected candidate will provide high-quality, professional medical care to our patients.

Key Responsibilities: -

As part of our team, you will:

• Provide comprehensive primary care services, including patient assessments, screenings, diagnoses, treatments, and preventive healthcare.
• Conduct physical examinations, obtain medical histories, and develop appropriate treatment plans.
• Prescribe and manage medications, create individualized care plans, and monitor patient outcomes in collaboration with the healthcare team.
• Educate patients on disease prevention, medication adherence, and overall health and wellness.
• Collaborate with physicians, behavioral health providers, and interdisciplinary teams to ensure coordinated, high-quality patient care.
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Coordinate referrals and consultations with specialists to support comprehensive patient care.
• Participate in team-based operations, including team meetings, clinician cross-coverage, and an after-hours on-call rotation.

Qualifications: -

· Master’s Degree or Doctorate from an accredited Nurse Practitioner program

· Current, unrestricted License as a Nurse Practitioner in the State of California

· Board Certification from the California Board of Registered Nursing

· Active and unrestricted DEA license

· Current BLS certification

· Minimum 2 years of Primary Care clinical work experience required

· Experience working in a community health center, FQHC, or underserved healthcare setting preferred.
