Licensed Practical Nurse (LPN)Eddy Visiting Nurse Association (VNA) is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to join our Intake Office team. This position plays a vital role in coordinating patient referrals and ensuring a smooth transition from referral receipt to scheduling and admission.The LPN will work collaboratively with an Intake Secretary and interdisciplinary team members to process referrals, communicate with healthcare providers, and ensure all required documentation is complete to support timely patient care.ResponsibilitiesReceive and process patient referrals for home care services.Communicate with referral sources, physician offices, hospitals, and other healthcare providers.Obtain and verify physician orders and required referral documentation.Review referral information for completeness and accuracy.Prepare referral packets for admitting clinicians and scheduling staff.Collaborate with the Intake Office team to facilitate efficient patient admissions.Maintain accurate documentation and data entry within electronic systems.Ensure compliance with organizational policies, regulatory requirements, and patient confidentiality standards.Provide exceptional customer service to patients, families, referral sources, and internal team members.QualificationsRequiredCurrent and active New York State Licensed Practical Nurse (LPN) LicenseStrong interpersonal and communication skillsExcellent computer and data entry skillsStrong organizational and time management abilitiesAbility to work effectively in a fast-paced team environmentPreferredOne (1) to two (2) years of acute care, home care, or nursing home experienceExperience coordinating patient care, referrals, or admissions processesWhat We OfferComprehensive orientation and training programSupportive and collaborative work environmentOpportunity to contribute to high-quality patient care within a respected home health organizationCompetitive compensation and benefits packageCareer growth and professional development opportunitiesWhy Join Eddy VNA?At Eddy VNA, our team is committed to delivering compassionate, high-quality care to patients throughout the community. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
