Transition Coordinator

This is a temporary position through 06/30/2027 with the possibility for extension.

POSITION PURPOSE:

The Transition Coordinator coordinates person-centered services for vulnerable individuals, helping them reach the highest possible level of independence by identifying the supports they need. The primary focus is helping individuals move from congregate settings to their own homes through a Minnesota Department of Human Services grant. This role also completes social service tasks that support each person's goals, including finding safe, affordable housing and arranging the services needed to maintain it.

NATURE AND SCOPE:

The Transition Coordinator reports to the Director of Community Supports, who reports to the Executive Vice President, who reports to the President. The Transition Coordinator should have a background of experience with mental health, traumatic brain injuries, ASD, intellectual and developmental disabilities, and related conditions. The Transition Coordinator must have the skills and knowledge to navigate complex service delivery systems and partner with other agencies and team members. Excellent interpersonal relationship skills are required. The job requires frequent contacts with clients, families, guardians, legal representatives, financial workers and other members of the interdisciplinary team to coordinate services for vulnerable individuals. The work environment requires frequent walking, standing, and sitting. Travel is required. The Transition Coordinator participates in frequent phone, deskwork and computer work.

JOB RESPONSIBILITIES:

Assist clients in locating and accessing housing and required waivered services, as well as medical, social, educational, and other needed services, regardless of funding source. Coordinate the plan of care. Serve as a liaison between the client and partnering agencies. Monitor services to help ensure the client's health and safety. Respond to and help resolve potential crisis situations. Participate as an interdisciplinary team member. Prepare reports, forms, and other information related to the overall plan of care (frequent writing/typing). Function as a role model and communicate effectively with clients, families, co-workers, and outside agencies. Work effectively with clients who have diverse cultural backgrounds and a range of medical, psychiatric, intellectual, and physical needs. Demonstrate positive, supportive, and cooperative conduct; foster cordial relationships and clear communication. Communicate clearly in writing and verbally; listen accurately and respond appropriately. Maintain knowledge of applicable rules, regulations, and state/county policies for the Home and Community Based Waiver (DD, CADI, and TBI). Complete all required documentation. Monitor and respond to emergencies according to established policies and guidelines. Assess client risk and respond appropriately to maintain client safety. Support an injury-free workplace by identifying and reporting unsafe working conditions. Participate in ongoing education related to housing, Minnesota Medicaid, and the DD, CADI, and TBI waiver programs. Stay current on topics related to the diagnoses of individuals served. Attend and complete all annual in-service requirements. Identify and respond to clients' emotional and social needs, recognizing that these supports are not psychotherapy. Perform tasks that may fall within Category II of infection control; there is no regular exposure to blood or high-risk body fluids.

QUALIFICATIONS:

High School Diploma required Bachelor's Degree in related field preferred Two (2) plus years experience working with persons with disabilities Excellent communication and interpersonal skills Excellent problem solving and organizational skills Telephone, computer, and communication skills Ability to read, write and speak English Ability to see and hear adequately to respond to emergency situations. Ability to travel to meet with clients and attend meetings, and if driving, holds a valid drivers license and maintains a safe driving record. Ability to sit, stand, reach, and bend.
