Registered Nurse (RN) - Hospice - $32-41 per hour

Job Title Registered Nurse (RN) Hospice – Part Time Location Traverse City, Michigan Job Summary Graham Healthcare Group (Residential Home Health and Hospice) is seeking a compassionate Registered Nurse to provide hospice care and manage patient care plans in Traverse City and surrounding areas. This part‑time position requires a weekend/after‑hour rotation and 20 hours per week. Responsibilities Collaborate with hospice care team to manage and deliver patient care plans. Provide nursing care to patients across outpatient and home settings. Maintain accurate and timely charting using EMR software. Travel to patient locations (~20% of work time) and manage your own schedule. Meet critical deadlines for documentation and care coordination. Qualifications Active RN license in the state of employment. Associate degree or higher from an accredited School of Nursing. Minimum two years of RN experience; hospice experience preferred. Valid driver’s license and ability to travel to patient locations. Strong time‑management and self‑management skills. Experience with tablets, mobile phones and EMR software.
