Licensed Practical Nurse - Long Term Care

Summary The LPN provides care and/or services appropriate to the age of the patients being served - and which is appropriate to the cognitive; emotional - and chronological maturation of the patient. Assesses data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age‑specific needs and to provide care needed as described in the services' policies and procedures. United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Written and spoken. Physical Standards: All applicants must meet the physical standards for the position; a physical examination may be required prior to employment. Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (ie. Puerto Rico) or District of Columbia. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed. Military health care education or training accepted by the licensing body in the jurisdiction will be considered as meeting the education requirements for VHA employment. Experience: Paid and unpaid experience, including volunteer work through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations, is considered. Credit will be given for qualifying experience. Grade‑specific requirements: GS‑3: None beyond the basic requirements. GS‑4: Six months of qualifying experience as an LPN or LVN, or graduation from an approved school and one year of experience in a licensed facility, or graduation from an approved school of at least 24 months duration. GS‑5: Completion of at least one year of qualifying experience at the GS‑4 level or equivalent. GS‑6: At least one year of additional qualifying experience at the GS‑5 level or equivalent, fully meeting all performance requirements for the GS‑5 LPN/LVN. Applicants must demonstrate knowledge and ability to provide a full range of practical nursing care, work with RN and MD/DO staff, educate patients/families, document medications, recognize emergencies, and apply customer service standards. The full performance level of this vacancy is GS‑6, but appointments may be at any grade between GS‑3 and GS‑6. Duties The licensed practical nurse performs duties including, but not limited to: Maintaining BCLS certification and current, active, unrestricted licensure. Delivering nursing care in long‑term care, dementia units, respite care, hospice, rehabilitation, and transitional care, supporting patients and their families. Providing practical nursing care, medications, and health education to patients. Collaborating with interdisciplinary teams to develop and execute patient treatment plans. Mentoring and pre‑training new employees, nurses, and nursing assistants. Responding to emergencies, including cardiac, disaster, and code yellow situations. Administering prescribed medications, performing diagnostic support duties, and following medication management systems such as Omnicell. Using nursing processes, critical decision‑making skills, and application of clinical guidelines. Managing unit cleanliness, reporting patient information, and ensuring compliance with policies. Co‑acting with family members, translating treatment plans, and supporting discharge goals. Participating in unit performance improvement, nursing CORE, and Medical Center committees. Work schedule: 15:00‑23:30 rotating weekends/holidays. Telework: Not available. Bargaining unit: Yes. Relocation/Recruits incentives: Authorized.
