Community Health Assistant

HarmonyCares

HarmonyCares, a physician-led home-based primary and palliative care organization, is seeking a Community Health Assistant (CHA) to support medically complex, home-limited patients. In this role, you’ll assist with care coordination, conduct in-home and telehealth visit support, gather vital patient information, and reinforce care plans under clinical supervision. Working within an interdisciplinary team, you’ll help reduce avoidable hospitalizations and improve outcomes. HarmonyCares offers a compassionate, mission-driven culture with opportunities for growth in a rapidly evolving healthcare environment.
