Hospice Access RN: Field & Triage Care

Providence Home Services Oregon is seeking an RN for Hospice Access, Field and Triage roles. The position includes visiting patients at home to assess eligibility and present hospice services, coordinating care with the IDT, and conducting both urgent and routine visits to current hospice patients.
