**Join Our Dynamic Team as a Physical Therapist in San Jose!** **Job Description:** - **Exciting Opportunities:** Work in diverse areas including San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, and Los Gatos. - **Flexible Travel:** Be prepared for travel outside your assigned area and possible weekend coverage to meet patient needs. - **Productivity Requirements (if Guarantee Applies):** - Achieve at least 27 points per week. - Coverage of 36 hours guaranteed. - Openness to patient assignments is essential for guarantee eligibility. - **Non-Guarantee Considerations:** - Flexibility to see patients beyond your service area may be required. - No guaranteed hours but the ability to refuse patients available. - Willingness to see Covid+ patients is necessary. - **Visit Types and Points System:** - OASIS ReCertification, Routine, Evaluation, and Discharge visits earn 1 point (1.33 hours). - OASIS SoC earns 1.5 points (2 hours). - **Comprehensive Compensation Includes:** - Driving/travel time. - Visit and documentation time. - Time spent communicating with patients and doctors. - **Required Credentials:** - Current CA PT License. - BLS Certification. - Valid Driver's License, Auto Insurance, and Registration. - Annual TB test, MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug screen, and Physical. - **Agency Compliance:** - Contract with your agency must replicate all details of this job description. - All requested time off (RTO) must be disclosed at submission. - **Minimum Experience Required:** 1 year. - **Shift Information:** - Day shifts available. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
