Key Responsibilities\n

As a Licensed Clinical Social Worker at Guideline Healthcare, you will:

• Conduct thorough bio-psychosocial evaluations of patients, identifying psychosocial problems that may hinder their recovery and overall health.

• Develop and implement personalized treatment plans that incorporate evidence-based practices, engaging with patients in both individual and group therapy settings.

• Work collaboratively with our medical staff to ensure integrated care for patients, enhancing their compliance and coordination of treatment.

• Provide education and resources to patients and families, empowering them to understand and follow through with recommended therapeutic and medical interventions.

• Maintain accurate documentation in patients’ medical records, adhering to the standards for frequency and content to uphold best practices in patient care.

• Participate in ongoing educational activities, keeping abreast of community resources that can facilitate patient advocacy and support systems.

• Regularly assess and address patient and family advocacy needs within the community to promote self-determination and resource access.

Patient Population / Client Focus\n

Your role as an LCSW will focus on diverse patient populations undergoing detox and recovery processes. This includes adults, adolescents, and, when appropriate, their families. You will provide targeted support to those dealing with complex psychosocial issues resulting from various mental health and substance use disorders, ensuring that each patient receives individualized care tailored to their unique circumstances.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Texas.

Preferred Experience\n

Compensation and Benefits\n
