California Licensed Bilingual Case Manager

As a bilingual Lead Case Manager in a full-time hybrid role, the candidate will provide comprehensive case management, mental health, and substance use services to individuals with persistent mental illness and/or substance use, ensuring personalized care and support within an integrated healthcare setting. Key responsibilities Serves as the primary contact for members and their support systems, coordinating care with a multidisciplinary team Assesses member needs and develops comprehensive Care Management Plans to address gaps in treatment and services Advocates for members, connects them to social services, and monitors treatment adherence while providing health promotion coaching Required qualifications Fluency in Spanish and a High School Diploma or GED; a Bachelor's degree in a related field is preferred Minimum of 2 years of experience working with Medi-Cal members in care management or mental health services Current California driver's license and acceptable driving record Knowledge of community resources and familiarity with trauma-informed care and harm reduction practices Exceptional communication skills and the ability to engage diverse populations effectively
