LPN Weekend Intake Coordinator

Job Title
Responsibilities:
- Saturday and Sunday. 8 or more hours each day available
- Process all referrals for Medicare Certified Home Health services
- Qualify all intake requests for both clinical appropriateness and financial reimbursement
- Ensure all intake data is accurate
- Articulate an understanding of Skilled Home Health services in all communication with referral sources and team members
- Exhibit excellent customer service
- Work closely with scheduling, Billing, and Quality departments
Qualifications:
- Licensed Practical Nurse.
