Key Responsibilities\n

As a Licensed Clinical Social Worker at our center, your daily responsibilities will include but are not limited to the following:

• Conduct detailed bio-psychosocial evaluations to identify patient challenges and barriers to treatment, ensuring appropriate resources are utilized to enhance health and wellness.

• Work collaboratively with a multidisciplinary team to design and implement patient-centered treatment plans, employing both individual and group therapy modalities as evidence-based practices.

• Document social work activities meticulously in patients' medical records to maintain compliance with departmental standards and to track patient progress accurately.

• Educate patients and their families regarding psychosocial recommendations and recovery plans, helping them understand and commit to these strategies for optimum functioning.

• Provide advocacy and support for patients and families, ensuring they have access to necessary community resources and support systems to enhance their care experience.

• Regularly assess patient and family needs, coordinating appropriate advocacy activities to empower clients in self-determination and improve social support systems.

Patient Population / Client Focus\n

Your role will focus on a diverse patient population, particularly those experiencing chronic health issues, mental illness, or environmental stressors related to hospice care. You will be tasked with addressing the unique psychosocial needs of these individuals and their families, tailoring your approach to their age, cultural background, and specific life circumstances.

Qualifications\n

To qualify for this role, candidates must possess:

• A Master’s Degree in Social Work from an accredited institution.

• A current Licensed Clinical Social Worker (LCSW) license issued by the State Board of Social Work Licensure.

Preferred Experience\n

Compensation and Benefits\n
