Description

• Assess patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to individual needs.
• Deliver targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
• Work collaboratively with nurses, physicians, case managers, and other rehab professionals to ensure optimal patient outcomes.
• Guide and educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercise routines.
• Document evaluations, treatment progress, and care strategies in compliance with facility and regulatory standards.
• Engage in interdisciplinary care team rounds and conferences to ensure cohesive patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist.
• CPR certification is preferred.
• Specialized certifications (neurological, geriatric) are advantageous but not mandatory.

Skills

• Prior experience strongly preferred.
