Job Description

We are seeking an experienced, adaptable Travel Registered Nurse (RN) - Long Term Care to join our team in Dubuque, IA . In this role, you will deliver high-quality nursing care to residents in a long-term care / skilled nursing facility, focusing on the daily management of chronic conditions and supporting frail older adults.

Key Responsibilities

• Perform comprehensive physical assessments and continuously monitor residents for acute changes in health status.

• Administer medications and manage IV therapy according to physician orders and facility protocols.

• Provide routine wound care and proactive skin integrity management, including pressure ulcer prevention.

• Document all care, treatments, and resident responses accurately using the facility's Electronic Health Record (EHR) system.

• Collaborate with an interdisciplinary team—including physicians, therapists, and social workers—to create and adjust individualized care plans.

• Educate residents and their families on disease management, medication administration, and safety planning.

• Facilitate smooth transitions of care within the facility or to higher-level medical care when necessary.

Qualifications & Requirements

Education & Licensure:

• Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.

• Licensure: Active, unencumbered RN license valid in the state of Iowa (or active Compact/NLC RN license).

Certifications:

• Basic Life Support (BLS) certification (Required).

• Certified Gerontological Nurse (CGN) or similar gerontology credential (Preferred).

Experience:

• Prior nursing experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting (Required).

• Direct experience with chronic disease management and geriatric patient care.

• Proficiency with EHR/EMR documentation systems and care coordination tools.

• Excellent clinical judgment, communication, and interpersonal skills.

Work Environment

• Work is performed in a fast-paced skilled nursing facility and retirement community alongside a multi-disciplinary healthcare team.

• Schedule consists of 12-hour day shifts (6:00 AM – 6:00 PM ).
