Your next nursing opportunity starts here! We are seeking a skilled Registered Nurse with strong OASIS experience for a full-time home health contract. This 13-week assignment offers the chance to make a meaningful difference by delivering high-quality care directly to patients in their homes. Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required
• Structured daily visit expectations

Your Key Responsibilities:

• Deliver attentive in-home nursing care and skilled interventions
• Complete comprehensive patient assessments and accurate, timely OASIS documentation
• Collaborate with physicians and interdisciplinary teams to implement individualized care plans
• Perform wound care, medication management, patient education, and chronic disease monitoring
• Maintain consistent communication regarding patient status and updates to the clinical team

Qualifications:

• Active RN license
• At least 1 year of recent experience as a home health RN
• Proficiency with OASIS required
• HomeCare HomeBase experience strongly preferred
• Strong organizational skills and ability to manage a structured schedule
• Reliable vehicle and valid drivers license
