Mobile Outreach Clinician - Homeless & Mental Health Support
Haymarket Center
Haymarket Center in Chicago is seeking a Mobile Outreach Specialist to provide outreach and clinical services to individuals experiencing homelessness, mental illness, and substance use. You will help triage and register clients and support individualized care planning. The role requires high school diploma with experience, strong assessment and case management skills, solid communication, mobility, and the ability to drive the agency van.
