Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical function, while aligning with department objectives
- Perform comprehensive evaluations and establish individualized treatment plans; implement evidence-based therapy interventions
- Monitor patient progress, adjust treatment plans as needed, and communicate changes to the care team
- Collaborate with an interdisciplinary team and help coordinate care across inpatient and outpatient settings
- Develop and communicate care plans, including discharge planning, to patients and appropriate parties
- Document clinical activities accurately and within established deadlines; supervise assigned assistants as applicable

Requirements
- Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent
- Physical Therapy licensure in the state of practice: Required
- CPR/BLS certification from an approved vendor: Required
- EPIC or similar electronic health record experience: Required
- Two years’ Physical Therapy experience: Preferred
- Ability to work in both outpatient and inpatient settings; no on-call duties required
