The hospital is looking for Physician - Emergency Medicine in Eau Claire, WI

JOB DESCRIPTION:

SUBMISSION REQUIREMENTS:

• ABEM/AOBEM Board Certificate or Board eligibility letter
• Must have a clean file/background no malpractice
• Procedure logs (last 2 years)

- Updated CV, drivers license, color photo, state medical license (including licensure held in other states), CSR, DEA, education certificates, 5yr malpractice history, need narrative & legal documentation if provider has claims, ECFMG if applicable, ABEM/AOBEM Board Certificate, Board eligibility letter, TB/PPD/Quantiferon /Chest x-ray (within 12 months), COVID, and FLU (seasonal).

AS A REMINDER:

• All candidates must have met the minimum shift availability listed on the job.
• CV must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most recent on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

Availability

• 1st possible date they would be available?
• How many shifts would they be able for per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc)?
• Any dates in the next 3-6 months they cant work?
• Any Holidays in the next 3-6 months they would be open to work.?

CERTIFICATION REQUIREMENTS

• ACLS
• BLS
• Board Certified
• PALS
• DEA (Drug Enforcement Administration) - Required
