This Clinical Pharmacist role offers the opportunity to deliver specialized pharmacy care in a dynamic hospital setting, working closely with interdisciplinary teams to optimize medication therapy and patient outcomes. Reporting to the Pharmacy Operations Manager or Clinical Pharmacy Manager, the pharmacist will be integral in clinical services, education, and sterile compounding within hospital units including emergency and intensive care.

Responsibilities:

• Provide expert clinical pharmacy services in assigned hospital units, collaborating with healthcare providers for optimal patient care.
• Review and independently verify medication orders for dosing, route, interactions, allergies, and therapeutic indications.
• Participate in multidisciplinary rounds, offering evidence-based medication therapy recommendations.
• Conduct medication reconciliation, patient counseling, and respond to drug information inquiries.
• Monitor patient medication profiles to identify and resolve drug-related problems.
• Support the PGY1 pharmacy residency program through precepting, educational presentations, evaluations, and feedback.
• Prepare sterile IV admixtures and compounded sterile products compliant with USP standards and facility protocols.
• Train and supervise pharmacy technicians and staff involved in sterile compounding.
• Mentor pharmacy students and contribute to clinical education initiatives.
• Engage in medication safety initiatives, quality improvement projects, and policy development.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications & Desired Experience:

• Doctor of Pharmacy (PharmD) from an accredited college of pharmacy.
• Active New York State pharmacist license in good standing.
• 1–3 years of hospital pharmacy experience, preferably in clinical settings like emergency or intensive care units.
• Completion of a PGY1 pharmacy residency.
• Board certification through the Board of Pharmacy Specialties (BPS) preferred.
• Demonstrated strong clinical knowledge and interest in pharmacotherapy and medication management.
• Experience and enthusiasm for teaching and precepting pharmacy residents and students.
