Full-Time Adult-Gerontology Nurse Practitioner at Health Care Center in Syracuse, NY

Position Overview

Health Care Center is seeking a dedicated and compassionate Adult-Gerontology Nurse Practitioner to join our full-time team in Syracuse, New York. The ideal candidate will play a pivotal role in delivering personalized, high-quality primary care to adult patients—particularly seniors enrolled in Medicare. This position requires a blend of clinical expertise, patient advocacy, and team collaboration to foster improved health outcomes and promote a culture of patient-centered care.

Work Environment

The position is 100% onsite at our home care facility located in Syracuse, NY. Within this supportive and collaborative environment, our Nurse Practitioners operate as integral members of a care team that is committed to delivering exceptional service. With resources dedicated to administrative support, including a medical scribe, our practitioners can focus on what they do best—caring for patients. The typical work schedule for this role is Monday through Friday, from 9:00 AM to 5:00 PM.

Key Responsibilities

• Provide exceptional primary care to patients through routine assessments, diagnosis, and management of acute and chronic conditions.
• Conduct office visits to address both routine and urgent medical issues while ensuring thoughtful, accurate, and specific documentation.
• Administer annual wellness visits and conduct health risk assessments, adopting a holistic approach that pays attention to the physical, mental, and social well-being of each patient.
• Facilitate care coordination by collaborating with specialists, testing facilities, and other healthcare providers to ensure comprehensive management of patient needs.
• Lead initiatives in population health, such as ensuring all eligible patients receive vital screenings or preventative care, fostering a culture of health and wellness.
• Assist the care team with phone triage, providing timely outreach to patients for follow-ups on care plans.
• Educate patients about their health conditions, treatment options, and pathways to better health, ensuring they are informed and engaged in their care.
• Participate in community outreach and promotional activities that reinforce our commitment to quality care.
• Conduct home visits when necessary, especially for patients with mobility challenges or those requiring bedside care.
• Report directly to the Medical Director while collaborating closely with operational leadership to align services and outcomes with organizational goals.

Patient Population / Client Focus

This role focuses primarily on older adults receiving care through Medicare, catering to a wide range of health concerns typical within this demographic, including chronic illnesses, medication management, preventative care, and lifestyle-related health challenges. By engaging deeply with this population, you will have the opportunity to make a significant impact on their health journey, helping them to remain independent and engaged in their community.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program is required.
• Hold national certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Possess an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 Coding, and Health Risk Assessments (HRAs) is essential.
• A strong inclination for teamwork and cross-functional collaboration.

Preferred Experience

• Experience in a value-based care setting or familiarity with population health methodologies is advantageous.
• History of effectively engaging with elderly patients and understanding their unique health needs.
• Proven ability to foster trust and rapport with patients through effective communication and compassionate care.

Compensation and Benefits

The salary range for this position is competitive, between USD 120,000 and 160,000 annually, commensurate with experience and qualifications.
