Work Environment

Your work will primarily be conducted in the homes of patients, allowing you to engage with each individual in a familiar setting. You will collaborate closely with a team that includes physicians, physician assistants, nurse practitioners, registered nurses, medical assistants, and dedicated medical scribes, all of whom work together to optimize patient outcomes through coordinated care. Key Responsibilities

• Deliver exceptional primary care through comprehensive health assessments and treatments.
• Conduct routine office and home visits for both preventive care and acute health issues.
• Administer annual wellness visits and facilitate health risk assessments, ensuring thorough documentation and understanding of patient health.
• Coordinate care with other healthcare providers, specialists, and community agencies to ensure seamless patient transitions.
• Lead population health initiatives, such as ensuring regular breast cancer screenings for eligible female patients.
• Assist the care team with phone triage, outreach, and patient engagement efforts to enhance communication and care delivery.
• Educate patients regarding their health conditions, treatment options, and care plans, empowering them to take an active role in their health.
• Participate in community outreach and promotional activities to raise awareness about our services.
• Document all patient interactions and care decisions in compliance with organizational and legal standards.
• Report directly to the Medical Director while collaborating with operational leadership for optimal care delivery.

Patient Population / Client Focus

Our primary focus is on Medicare-eligible adults, particularly those with complex health needs. We tailor our services to enhance their health and well-being, preventing frequent hospital admissions. The nurturing approach to care emphasizes building strong patient-provider relationships, ensuring that patients feel valued and supported every step of the way.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or successful completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Active and non-probationary Nurse Practitioner license in New York State.
• Current DEA registration for the prescribing of medications.
• A robust background in primary care settings, including experience in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and conducting health risk assessments.
• Exceptional teamwork skills with an eagerness to collaborate across disciplines.

Preferred Experience

• Previous experience in home care or community health settings.
• Proficiency in electronic health record (EHR) systems and digital communication tools.
• Experienced in managing chronic conditions and understanding the unique needs of the older adult population.
