State Health, LLC is seeking a dedicated Nurse Practitioner specializing in Internal Medicine to join our team in Syracuse, New York. This full-time position allows health professionals to operate within a vital care network, delivering exceptional primary care to our adult patients, primarily those enrolled in Medicare. Work Environment

This role is situated onsite, primarily providing home care to patients in the Syracuse area. You will be part of a dynamic healthcare team which includes physicians, nurse practitioners, physician assistants, medical assistants, and care managers. Additionally, our support infrastructure includes dedicated medical scribes to help you with documentation, ensuring you can focus on comprehensively addressing patient needs.

Key Responsibilities

• Provide top-notch primary care to adult patients, ensuring continuity and quality of care.
• Perform thorough office visits addressing routine and acute health concerns.
• Administer annual wellness visits and health risk assessments, maintaining meticulous documentation and emphasizing a holistic health perspective.
• Coordinate care with specialists, testing facilities, and other agencies to ensure all patient needs are met.
• Lead population health initiatives alongside the Care Team to ensure preventive measures are in place, such as regular screenings.
• Engage in patient education, explaining health conditions and treatment plans effectively.
• Perform phone triage and follow-up outreach to manage patient care proactively.
• Conduct home visits when necessary to provide direct care in the patient's living environment.
• Contribute to promotional activities that enhance the organization’s visibility in the community.

Patient Population / Client Focus

You will primarily be serving older adults enrolled in Medicare, many of whom have complex health needs. Our focus is on providing personalized care that aligns with the patients' lifestyles and preferences, facilitating their autonomy while ensuring comprehensive care management. Our model is specifically designed to empower patients, keeping them as healthy and engaged as possible.

Qualifications

• A Master's Degree in Nursing, a Doctor of Nursing Practice, or successful completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• A thorough understanding of HCC (Hierarchical Condition Categories) documentation as well as ICD-10 coding.

Preferred Experience

• Experience in primary care settings related to internal medicine, geriatrics, or family medicine.
• Familiarity with health risk assessments (HRAs) and population health management.
• Bilingual proficiency can be an asset, enabling us to serve a broader patient demographic.

Compensation and Benefits

The salary range for this position is between USD 115,000 and 150,000 annually, depending on your experience and expertise. We also offer a performance-based quarterly bonus tied to quality metrics, providing an additional incentive for outstanding patient care.
