Family Medicine Nurse Practitioner - Full-Time Opportunity in Home Care

Position Overview

Brightwood Orthopedic and Family Medicine Care is seeking dedicated and skilled Nurse Practitioners to join our team in Syracuse, New York. This full-time position operates onsite within a home care setting, offering the unique opportunity to work closely with patients in their own environment. As a Nurse Practitioner, you will be instrumental in delivering exceptional primary care services aimed at improving the health and well-being of older adult patients on Medicare.

Work Environment

This position is set in a dynamic and collaborative environment where you will work alongside a multidisciplinary care team. Your team includes physicians, nurse practitioners, physician assistants, medical assistants, a dedicated medical scribe, registered nurses, and care managers. Together, you will emphasize care within the clinic, honing in on primary care, care coordination, and population health management. The work schedule is Monday through Friday from 8:00 AM to 5:00 PM, providing a structured yet rewarding setting to impact patients’ lives positively.

Key Responsibilities

• Deliver exceptional primary care through comprehensive assessments, care planning, and follow-up.
• Conduct office visits for both routine and acute health issues, ensuring each patient receives personalized attention.
• Administer annual wellness visits and health risk assessments, incorporating a holistic view of each patient's health.
• Coordinate care effectively with other healthcare providers, specialists, testing facilities, and community agencies.
• Lead initiatives in population health to ensure eligible patients receive necessary screenings and care, such as evidence-based breast cancer screenings.
• Engage with the care team to provide efficient phone triage, outreach, and patient education regarding health conditions and treatment plans.
• Participate in promotional activities that may include community outreach and wellness events.
• Conduct necessary home visits to better serve patients who may have mobility or transportation challenges.
• Report to the Medical Director and work closely with operational leadership on addressing clinic needs and patient care strategies.

Patient Population / Client Focus

Your primary focus will be on Medicare-eligible older adults, ensuring they receive personalized and accessible healthcare. The innovative care model revolves around maintaining and enhancing their quality of life through proactive health management and coordination of care directly in the communities they inhabit. This individualized attention allows for deeper relationships with patients, promoting an effective and empathetic healthcare experience.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an equivalent Nurse Practitioner Certificate Program.
• National certification as a Family or Adult-Gerontology Primary Care Nurse Practitioner, achieved through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license, accompanied by an active DEA license.
• Strong background and experience in primary care domains such as geriatrics, internal medicine, or family medicine.
• Proficient understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and health risk assessments.
• A passion for teamwork and innovative population health models, with a desire to collaborate effectively across disciplines.
• Bilingual proficiency is a significant plus, enabling better communication with diverse patient populations.

Preferred Experience

• Experience working in a primary care setting, particularly with older adults, is preferred.
• Demonstrated ability to lead population health initiatives and manage patient outcomes effectively.
• Previous involvement in care coordination or community health outreach projects is highly desirable.

Compensation and Benefits

The salary for this position ranges from $115,000 to $150,000 annually, reflective of experience and expertise. In addition to competitive compensation, we offer a comprehensive benefits package that includes:
