Work Environment

As a Nurse Practitioner at ProMedication Counseling Center, you will work onsite in a supportive assisted living facility in Syracuse. The work schedule is Monday to Friday from 9 AM to 5 PM, allowing for a balanced work-life integration. Our facility is equipped with modern medical technologies and resources that empower our team to provide top-notch patient care within a collaborative interdisciplinary framework.

Key Responsibilities

In this role, you will be responsible for a multitude of tasks, including:

• Conducting comprehensive patient assessments to identify health needs and develop appropriate care plans.
• Managing patients through acute medical situations or exacerbations of chronic conditions, which may include interpreting diagnostic tests and implementing timely interventions.
• Overseeing short-term rehabilitation episodes for residents, ensuring effective recovery strategies tailored to individual needs.
• Maintaining precise and thorough documentation, including the use of ICD-10 coding for accurate diagnosis reporting.
• Collaborating closely with an interdisciplinary team of healthcare professionals to provide integrated care.
• Facilitating advanced care planning conversations with patients and their families to discuss goals and preferences for care.
• Providing counseling and education to patients and their caregivers, helping them to navigate their health conditions and treatment options.

Patient Population / Client Focus

Our facility caters to a diverse group of residents, ranging from those with chronic illnesses requiring ongoing management to patients recovering from acute medical conditions. You will be working with older adults who may have complex behavioral, social, and functional needs, ensuring that each resident receives personalized care tailored to their unique circumstances.

Qualifications

The following qualifications are essential for this position:

• Current certification as a Nurse Practitioner.
• Active nursing license in the state of New York.
• Bachelor’s degree in nursing (Master’s degree preferred).
• A minimum of 3 years of clinical experience, ideally within a hospital or long-term care setting.

Preferred Experience

• Experience in assisted living, skilled nursing, acute care, emergency, or urgent care settings.
• Proven ability to address the medical needs of patients with diverse behavioral and functional challenges.
• Strong understanding of internal medicine and chronic illness management, along with advanced illness care and end-of-life discussions.
• Proficiency with electronic medical records (EMR) and other healthcare technology systems.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience, along with a comprehensive benefits package.
