Work Environment

This position is based onsite, primarily within patients' homes, which allows you to build strong relationships and foster trust with the families you serve. Key Responsibilities

• Conduct thorough and comprehensive health assessments and physical evaluations for patients of all ages.
• Order, perform, and interpret diagnostic tests such as lab work and imaging studies to make informed clinical decisions.
• Collaborate closely with a multidisciplinary healthcare team to design and implement patient-centered care plans tailored to individual needs.
• Prescribe medications and manage treatment regimens, ensuring adherence to state and federal regulations.
• Provide health education and counseling to patients and their families on managing chronic conditions and achieving wellness goals.

Qualifications
