Work Environment

This role is based in a vibrant assisted living facility where you will work closely with a multidisciplinary team to coordinate and deliver care. Our facility prioritizes a collaborative approach, fostering strong relationships between healthcare professionals and the patients we serve. With an average workweek of approximately 40 hours, your schedule will typically run from 9:00 a.m. to 6:00 p.m., though occasional evening clinics may extend to 8:00 p.m., and participation in community outreach and health fairs may take place on weekends.

Key Responsibilities

• Perform comprehensive health assessments and health screenings, offering tailored care that considers the unique health histories and needs of patients.
• Diagnose and manage a range of acute and chronic health conditions, providing preventive care, health promotion, and patient education to empower individuals in managing their health.
• Collaborate with local community organizations to design and implement effective health education programs aimed at enhancing community health awareness.
• Advocate for patient rights and equitable access to healthcare services, ensuring culturally sensitive care tailored to diverse populations.
• Coordinate patient referrals and follow-up care to ensure seamless continuity of healthcare services.
• Maintain accurate and thorough medical records in compliance with healthcare regulations and organizational standards, utilizing EMR systems to document patient care effectively.
• Engage in community outreach events to raise health awareness and provide vital health screenings.
• Stay informed about current trends in community health, evidence-based practices, and healthcare policies relevant to our patient population.

You will primarily serve individuals and families residing within the assisted living facility, offering a holistic approach to healthcare that encompasses physical, emotional, and social well-being. Qualifications

• Current certification as a Family Nurse Practitioner is required.
• Must possess an active state nursing license, allowing for practice in New York State.
• A Master's degree in nursing from an accredited program is essential.

Preferred Experience

Compensation and Benefits

The salary range for this position is competitive, offering between $180,000 and $200,000 annually, commensurate with qualifications and experience.
