Work Environment

This Nurse Practitioner position is onsite and entails working directly in patients' homes across Syracuse, New York. Key Responsibilities

• Conduct thorough assessments and formulate diagnoses for patients experiencing a spectrum of medical conditions, from chronic illnesses such as diabetes and hypertension to acute presentations requiring immediate management.
• Develop and execute comprehensive, evidence-based treatment plans in conjunction with interdisciplinary teams, ensuring that care is tailored to each patient’s specific needs and preferences.
• Utilize electronic medical record (EMR) systems to maintain accurate patient documentation, track progress, and manage health information effectively, enhancing communication and continuity across care teams.
• Order, interpret, and manage the results of diagnostic tests, such as lab work and imaging studies, to inform clinical decision-making and adjust treatment plans as necessary.
• Educate patients and their families about disease processes, medication management, and effective strategies for self-care, including lifestyle modifications to improve health conditions.
• Engage in routine interdisciplinary rounds to collaborate with physicians, nurses, social workers, and other healthcare professionals, aiming to produce well-coordinated care plans that address all aspects of a patient’s health.
• Participate actively in quality improvement initiatives to enhance patient care practices based on the latest evidence and ensure compliance with healthcare regulations.
• Stay current with ongoing developments in medical guidelines and home healthcare trends to continually enhance your own practice and patient education efforts.

Patient Population / Client Focus

The primary focus of this role is on patients who require ongoing medical management in home care settings. This includes individuals with chronic conditions, post-operative patients needing recovery assistance, and those in need of palliative care. Your ability to provide compassionate, patient-centric care will be crucial in optimizing their health outcomes and improving quality of life.

Qualifications

• A current Nurse Practitioner certification and active state nursing license in New York are required.
• A Bachelor's degree in nursing or an equivalent qualification is necessary for application consideration.
• At least four years of clinical experience as a Nurse Practitioner, with a focus on home care or community health experience highly preferred.
• Exceptional communication skills are essential for effective patient education and interdisciplinary collaboration.
• Strong analytical and problem-solving abilities are required to assess patient conditions and develop appropriate treatment strategies.

Preferred Experience

Compensation and Benefits

Gotham Enterprises Ltd offers a competitive salary range of $120,000 to $220,000 annually, commensurate with your experience and qualifications.
