Work Environment

This role is based onsite in Garland, Texas, with services provided through house calls. Key Responsibilities

• Conduct thorough health assessments and screenings for patients during house calls and in-clinic consultations, focusing on both acute and chronic conditions.
• Diagnose and establish management plans for various medical issues, while promoting preventative care and health education among patients.
• Develop and implement health education programs in collaboration with community organizations, addressing specific community health needs and trends.
• Advocate for patients’ rights and work towards increasing accessibility to healthcare services, especially for marginalized communities.
• Coordinate referrals and ensure follow-up care to maintain continuity for all patients, collaborating with other specialists as needed.
• Maintain comprehensive and accurate electronic medical records adhering to legal and regulatory requirements, ensuring efficiency in patient care.
• Participate actively in community outreach events and health fairs, promoting wellness and providing essential health screenings.
• Stay informed on the latest evidence-based practices, emerging community health issues, and relevant health policies to continuously enhance service delivery.

Patient Population / Client Focus

In this role, you will primarily serve diverse populations residing in Garland and surrounding areas. We aim to improve healthcare outcomes for underserved communities and vulnerable groups. Qualifications

• Current certification as a Nurse Practitioner, enabling you to provide high-quality patient care.
• An active nursing license in the state of Texas.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, with a preference for individuals who have worked in community health or public health settings.
• Exceptional interpersonal skills and the ability to communicate effectively with a diverse patient population, fostering trust and rapport.

Preferred Experience

• Previous experience in concierge medicine or similar healthcare delivery models is highly desirable.

Compensation and Benefits
