Work Environment

This role will be primarily onsite, involving direct patient interactions in their homes or wherever they feel comfortable. You will be part of a collaborative healthcare team comprising physicians, registered nurses, and various specialists. Key Responsibilities

• Conduct thorough assessments of adult patients, including detailed medical histories, physical exams, and necessary diagnostic tests to develop a comprehensive understanding of their health status.
• Diagnose, treat, and manage a wide range of acute and chronic health conditions, coordinating closely with physicians and specialists to ensure the best possible outcomes for patients.
• Formulate and implement individualized treatment plans, employing evidence-based practices that integrate patient preferences and goals for effective disease management.
• Prescribe appropriate medications and treatments, carefully monitoring patient progress and adjusting plans as needed to optimize results.
• Deliver education to patients and their families on managing chronic illnesses, promoting healthy lifestyle changes, and understanding preventive measures to enhance overall well-being.
• Perform in-home procedures within your scope of practice, prioritizing patient comfort and safety while adhering to stringent clinical standards.

Qualifications

• Current certification as a Nurse Practitioner, with a valid and active state nursing license.
