Key Responsibilities

• Conduct comprehensive health assessments and manage patient care for individuals dealing with both acute and chronic health issues in their homes.
• Develop individualized treatment plans utilizing evidence-based practices in collaboration with Physicians and specialists.
• Order and interpret diagnostic tests, as well as prescribe medications relevant to patient diagnoses and needs.
• Provide thorough education to patients and their families regarding health management, medication strategies, and supportive care post-discharge.
• Participate actively in interdisciplinary healthcare team meetings, contributing insights that enhance patient outcomes while advocating for wellness initiatives.
• Maintain accurate, thorough, and timely electronic medical records through advanced clinical documentation software.
• Engage in quality improvement initiatives aimed at enhancing patient care services and operational efficiency.
• Keep abreast of ongoing changes in healthcare legislation, technology advancements, and treatment innovations to ensure compliance and quality service delivery.

Qualifications

• Current certification as a Nurse Practitioner with an active state license in Texas.
• A bachelor's degree in nursing or a related field from an accredited institution.

• Outstanding interpersonal and communication skills are necessary to establish rapport with patients and collaborate effectively with healthcare teams.
