Experienced Family Nurse Practitioner - Assisted Living Facility

Work Environment

This position is based onsite at our Assisted Living Facility, where you will work closely with nursing staff, physicians, and the administration team to provide coordinated care. The work schedule is primarily from 8:00 a.m. to 5:00 p.m. on weekdays, with occasional evening clinics extending until 8:00 p.m. Participation in weekend outreach events or health fairs may occasionally be required, making flexibility an essential component of this role. Key Responsibilities

• Conduct comprehensive physical assessments for residents to evaluate health status and determine necessary interventions.
• Diagnose and manage both acute and chronic health conditions, ensuring preventive care methodologies are employed for overall health promotion.
• Order, interpret, and act on diagnostic tests, using evidence-based practices to guide patient care decisions.
• Develop personalized care plans while collaborating with interdisciplinary teams to address diverse patient needs.
• Provide health education and counseling to patients and their families, enhancing understanding of treatment options and self-care strategies.
• Advocate for patients' rights, ensuring access to healthcare services and addressing barriers to care.
• Maintain comprehensive and accurate medical records, meeting all regulatory and ethical standards.
• Engage in community outreach initiatives, participating in health fairs and public health campaigns to promote wellness.
• Continuously update your knowledge of current health trends and developments in nursing practice relevant to our patient population.

Patient Population / Client Focus

This role primarily focuses on the elderly population residing in our Assisted Living Facility. Consequently, you will encounter a diverse range of health concerns, including chronic illnesses, cognitive impairments, and complex healthcare needs. Understanding the unique challenges that elderly patients face, you will provide thoughtful, culturally sensitive care that involves family engagement in treatment planning.

Qualifications

• Successful completion of a Master’s degree in Nursing from an accredited institution.
• Current Nurse Practitioner certification and an active nursing license in Texas.
• Strong interpersonal and communication skills, with a commitment to providing patient-centered care to diverse populations.
• Dedication to collaborating effectively with healthcare professionals and community organizations.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in community health, geriatrics, or similar settings.
• Experience working within an assisted living or long-term care environment is advantageous.
• Proficiency with electronic health record (EHR) systems, ensuring accurate documentation and management of patient information.

Compensation and Benefits
