Full-Time Primary Nurse Practitioner - Assisted Living Facility

Work Environment

As a Primary Nurse Practitioner at Gotham Enterprises, you will work in a state-of-the-art assisted living facility that promotes collaboration and teamwork among healthcare professionals. The facility is designed to create a safe and comfortable atmosphere, equipped with the latest medical technologies and resources necessary to support your practice. You will enjoy a structured work schedule, typically comprising 12-hour shifts that equate to 36 to 40 hours weekly, allowing you to balance your professional commitments with personal life.

Key Responsibilities

• Conduct thorough and comprehensive assessments of residents to diagnose acute and chronic medical conditions, ensuring each resident's healthcare needs are met promptly.
• Develop, implement, and modify patient-centered care plans using evidence-based practices, in collaboration with our team of physicians, nurses, and allied health professionals.
• Order and interpret diagnostic tests, and prescribe appropriate medications and therapies to ensure effective treatment outcomes.
• Provide education to residents and their families on disease management, medication adherence, and post-discharge care strategies tailored to each resident’s condition and lifestyle.
• Engage in interdisciplinary team meetings and rounds, contributing insights and strategies that enhance the holistic care provided to residents.
• Maintain detailed and accurate electronic medical records (EMR) to document assessments, care plans, and adjustments to treatment, as well as review and update records according to regulatory standards.
• Initiate and participate in quality improvement initiatives, continuously working to enhance patient outcomes and implement evidence-based practice guidelines.
• Stay abreast of current healthcare trends, treatments, and regulations to ensure compliance and optimal patient care delivery.

Patient Population / Client Focus

Your primary focus will be on a diverse group of residents in an assisted living setting, many of whom may have complex healthcare needs, co-morbidities, and varying degrees of autonomy. You will be instrumental in fostering supportive relationships with residents and their families, ensuring they feel heard and valued as partners in their care journey.

Qualifications

• A current Nurse Practitioner certification and an active state nursing license for Texas.
• A Bachelor’s degree in nursing; a Master’s degree in nursing or a related field is preferred.
• A minimum of four years of clinical experience, ideally in an assisted living or long-term care setting.
• Exceptional communication skills that foster teamwork and collaboration.
• Strong critical thinking and clinical judgment abilities to make informed decisions regarding patient care.

Preferred Experience

While a minimum of four years of related experience is essential, candidates with specialized training or experience in geriatrics, chronic illness management, or acute care will be prioritized. Familiarity with electronic health record systems and experience in quality improvement initiatives are advantageous.

Compensation and Benefits

• A competitive annual salary range of USD 120,000 – 220,000, commensurate with experience and qualifications.
