Key Responsibilities

• Conduct thorough assessments and accurately diagnose both acute and chronic medical conditions, ensuring timely and effective treatment.
• Formulate and implement individualized treatment plans, demonstrating autonomy and clinical judgement in patient care decisions.
• Order and interpret diagnostic tests, including blood work and imaging studies, to guide clinical decision-making.
• Prescribe medications and therapies in compliance with state regulations, while closely monitoring patient responses to treatment.
• Educate patients and their families on health management, preventive care strategies, and lifestyle modifications to enhance well-being.
• Collaborate with physicians, nursing staff, and social workers to coordinate holistic patient care and promote a team-oriented approach to health services.
• Maintain meticulous and confidential medical records in our electronic health record system, adhering to all regulatory standards.
• Actively participate in quality improvement initiatives and patient safety programs to optimize care processes and health outcomes.

Patient Population / Client Focus

Our Nurse Practitioner will cater to a diverse patient population, primarily consisting of elderly residents within our assisted living facility. This role will require a strong understanding of geriatric health issues, including chronic disease management, dementia care, and the emotional needs of aging individuals. Qualifications

• Active and current certification as a Nurse Practitioner.
• State licensure to practice in Texas, with a clean professional record.
• A Bachelor's Degree in Nursing; a Master's Degree in Nursing from an accredited program is preferred.
• A minimum of 3 years of clinical experience as a Nurse Practitioner, particularly in geriatrics or chronic illness management.

Preferred Experience

Compensation and Benefits

In recognition of your skills and expertise, we offer a competitive salary range of USD 120,000 to 220,000 annually, commensurate with your experience and qualifications.
