Experienced Nurse Practitioner in Cardiology - Home Care Setting

Work Environment

As a Nurse Practitioner within the home care setting, you will work closely with a multidisciplinary team, including Cardiologists, Registered Nurses, and administrative support staff. Key Responsibilities

• Collaborate with a team of Cardiologists to provide exceptional care for patients experiencing cardiovascular issues, ensuring thorough evaluation and treatment plans.
• Conduct comprehensive evaluations, including taking patient histories and performing physical exams to assess cardiac health.
• Manage and monitor chronic conditions such as hypertension and hyperlipidemia, developing individualized care plans tailored to each patient.
• Provide outpatient-only Electrophysiology services, including evaluating abnormal electrocardiograms and managing device patients.
• Diagnose and manage acute heart conditions, including Coronary Artery Disease, Peripheral Artery Disease, and Congestive Heart Failure, ensuring timely interventions.
• Administer stress testing as needed, interpreting results and adjusting treatment plans accordingly.
• Referring patients to specialists within the group when necessary and facilitating continuity of care.
• Educate patients and their families about cardiac health, treatment options, and lifestyle modifications.

Patient Population / Client Focus

Your patient population will primarily include individuals diagnosed with various cardiovascular conditions, who require continuous management and monitoring. This role entails engaging with a diverse group of patients, ranging from those with chronic disease management needs to acute care cases, ensuring that each patient feels supported and receives personalized attention in their own home.

Qualifications

• Education: Must possess a Master’s or Doctorate Nurse Practitioner degree from an accredited institution.
• Licensing: Valid and current Nurse Practitioner license in New York State, with certification in a relevant specialty.
• Accredited Training: Completion of an accredited Nurse Practitioner program that is aligned with departmental needs, such as Cardiology.

Preferred Experience

• Prior experience in outpatient Cardiology, particularly in managing arrhythmias, electrophysiology, or similar fields.
• Experience with advanced cardiac care practices, acute condition management, and stress testing procedures.
• Strong communication skills and a collaborative mindset to work effectively within the healthcare team.

Compensation and Benefits

The annual salary for this position is competitive, ranging from USD 120,000 to 130,000, commensurate with experience. In addition to a rewarding salary, we offer a comprehensive benefits package that includes:
