The Gundersen Health and Wellness Clinic is actively seeking a dedicated and knowledgeable Nurse Practitioner (NP) to join our esteemed Center for Advanced Cardiac Care (CACC) team in Syracuse, New York. As a key player in our outpatient practice, you will work under the guidance of the Chief Nurse Practitioner and in collaboration with the CACC Medical Director. This position emphasizes a patient-centered approach, where you will engage with a multidisciplinary team to enhance cardiovascular health among our patients.

Work Environment

In this full-time role, you will be functioning primarily in an onsite environment, engaging directly with patients and families within our state-of-the-art facility. Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing comprehensive physical examinations.
• Order and interpret diagnostic tests to formulate informed diagnoses and manage both acute and chronic health concerns, aimed at optimizing patient outcomes.
• Develop and implement individualized, goal-directed care plans that include health maintenance strategies and interventions aligned with established clinical protocols.
• Maintain open communication with physicians and other healthcare providers to adapt treatment plans in response to patient progress and emerging needs.
• Educate patients and caregivers on disease management and preventive measures, ensuring they understand their treatment and any necessary lifestyle modifications.
• Coordinate hospital admissions and collaborate with inpatient teams to ensure effective discharge planning and follow-up care.
• Utilize electronic medical records (EMR) to document all patient interactions and treatment plans accurately.
• Engage in research protocols by reviewing and integrating findings into clinical practice to improve care delivery continuously.
• Participate in team meetings that focus on care coordination, addressing complex cases with input from social workers, nutritionists, pharmacists, and palliative care teams.
• Contribute to maintaining a safe, effective healthcare environment and uphold the standards of practice set forth by regulatory bodies.

Patient Population / Client Focus

The Nurse Practitioner will focus on a diverse patient population, including individuals with advanced heart failure, pulmonary arterial hypertension, and post-heart transplant patients. This specialized care requires a thorough understanding of complex cardiac conditions and the ability to manage patients undergoing advanced therapies, including outpatient intravenous and inhaled medication support, as well as mechanical circulatory support, while fostering a strong rapport with patients and their families.

Qualifications

• A Master’s Degree in Nursing from an accredited program with a specialization in Nurse Practitioning.
• A valid New York State Registered Professional Nurse and Nurse Practitioner license.
• Certification from a recognized NP Board.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), and possess a valid Drug Enforcement Administration (DEA) registration and National Provider Identifier (NPI).
• Familiarity with advanced healthcare information systems, particularly electronic medical records (EMR), Microsoft Word, PowerPoint, and Excel.

Preferred Experience

• Minimum of two years of experience in a hospital or outpatient cardiology setting.
• Experience working with complex patient populations, particularly those facing advanced cardiac therapies.
• Demonstrated ability to navigate challenging care scenarios with compassion and clinical expertise.

Compensation and Benefits

The competitive salary for this position ranges from USD 120,000 to 220,000 annually, commensurate with experience and qualifications.
