Work Environment

This full-time position is primarily onsite within a school-based clinic setting, allowing for convenient access to a diverse patient population. You will enjoy a Monday through Friday schedule, with hours from 8:00 AM to 5:00 PM, providing a stable work-life balance. Key Responsibilities

• Deliver comprehensive primary care services, conducting routine office visits for both acute and chronic health issues.
• Administer thorough Annual Wellness Visits and Health Risk Assessments, ensuring detailed, accurate, and thoughtful documentation aligned with patient needs.
• Lead care coordination efforts, engaging with other healthcare providers, specialists, and testing facilities to facilitate seamless patient care.
• Guide population health initiatives by ensuring adherence to preventative care guidelines, such as cancer screening protocols.
• Assist the care team with phone triage and outreach efforts to improve patient engagement and access to services.
• Educate patients about their health conditions, crafting individualized care plans that empower them to take charge of their health.
• Participate in promotional activities to foster a community-centric healthcare approach.
• Conduct patient home visits as needed to offer flexible care solutions and address individual needs.
• Report directly to the Medical Director, maintaining close collaboration with operational leadership for optimal service delivery.

Patient Population / Client Focus

Your primary focus will be on Medicare-eligible adults, many of whom may face unique health challenges due to age and comorbid conditions. By providing personalized and empathetic care, you will play an essential role in enhancing patients' quality of life and ensuring they understand their health directions and treatment plans. The aim is to keep our patients happy, healthy, and engaged in their own care processes.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Nurse Practitioner in Family, Adult, or Adult-Gerontology Primary Care through AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Current DEA license to prescribe medications.
• A solid foundation in primary care, specifically within internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is highly desirable.
• A passion for teamwork and collaboration in a cross-functional healthcare environment.

Preferred Experience

• Experience in outpatient settings, particularly with older adult populations.
• Bilingual proficiency is a plus, enhancing communication with diverse patient populations.

Compensation and Benefits
