Key Responsibilities

As a Nurse Practitioner specializing in Internal Medicine, you will be responsible for a wide range of clinical duties, including:

• Conducting comprehensive initial patient histories and performing thorough physical examinations to assess health conditions accurately.
• Developing, documenting, and communicating individualized care plans, while actively following up to ensure patients are appropriately screened and prepared for treatments.
• Participating in daily patient rounds and updating the healthcare team regarding patient progress and needs.
• Prescribing medication and ordering laboratory tests as well as diagnostic evaluations, ensuring all decisions align with best practice standards.
• Interpreting test results, providing critical insights, and making evidence-based decisions in partnership with the healthcare team.
• Collaborating with attending physicians, residents, nursing staff, and various healthcare providers to ensure the highest quality of patient care.
• Providing detailed discharge summaries, prescriptions, and referrals on patient charts to support continuity of care.
• Delivering clear, concise post-discharge instructions to patients to enhance understanding and adherence to care plans.
• Performing any additional duties deemed necessary to support the function of the office, involving a commitment to continuous improvement and patient safety.

Patient Population / Client Focus

The Nurse Practitioner will engage primarily with adult patients exhibiting a range of chronic and acute illnesses commonly seen in Internal Medicine. Our patient population is diverse, and you will have the opportunity to work with individuals from various backgrounds, managing a spectrum of health challenges and promoting preventive care strategies.

Qualifications

To be considered for this role, candidates must meet the following requirements:

• A Master’s Degree in a Nurse Practitioner Program is required, along with National Board Certification in the relevant specialty area.
• A current and valid license to practice as a Nurse Practitioner in New York State, as well as a Registered Professional Nurse license in New York State.
• Possession of a collaborative practice agreement with designated protocols filed with the NYS Department of Education or a collaborative relationship attestation as per the Nurse Practitioner Modernization Act of 2015.
• Current Basic Life Support (BLS) certification is mandatory.

Preferred Experience

While not mandatory, candidates with experience in managing patients within an outpatient setting, as well as familiarity with electronic health records systems and telehealth platforms, will find themselves well-suited for this role. Experience in interdisciplinary collaboration and performance improvement initiatives is highly valued.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 150,000 to USD 160,000 per year, commensurate with experience.
