Clinical Care Nurse

Conviva Senior Primary Care is seeking a Clinical Care Nurse to provide compassionate, coordinated care for adults 65+. In this role you’ll deliver direct patient care, perform assessments, administer medications, and support chronic disease management in a primary care setting. You’ll educate patients and families, coordinate with an interdisciplinary team, and promote prevention, wellness, and safety. The nurse will use electronic health records, follow evidence-based protocols, and contribute to quality improvement initiatives in a collaborative, patient-first environment.
