Social Service Worker

Job Description Gather psychosocial information to contribute to the development and implementation of a comprehensive care plan that delivers extraordinary patient care while remaining mindful of costs and resources. Collaborate with patients, their families or significant others, healthcare providers, insurers, community resources, and other stakeholders. The SSW will execute these duties under direct clinical supervision. Position Details Shift: 8:00 AM – 4:30 PM on Tuesdays and Thursdays. No weekend or holiday requirement unless the holiday falls on a scheduled shift. Department: Behavioral Health and an Access Center. Minimum Qualifications State of Utah SSW license. Three years of experience working with behavioral health patients in a behavioral health care setting. Excellent interpersonal and communication skills. Ability to adapt quickly when the need arises. Knowledge of available behavioral health community resources. Preferred Qualifications Post–high‑school education in a related field of study. Experience with discharge planning for behavioral health patients.
