Lead Physical Therapist - In-Home Health (Contract)

TEEMA Solutions Group is seeking a Physical Therapist (PT) Lead for a 13-week contract in Northbrook, IL. The role requires 1+ year of Home Health experience and HCHB experience. Responsibilities include providing in-home evaluations, managing a patient caseload in the Chicago and Lake Forest territories, and coordinating care with an interdisciplinary team.
