Physical Homecare Therapist

Position Summary A Home Health Physical Therapist provides skilled rehabilitation services in patients’ homes to improve mobility, strength, balance, and functional independence. This role focuses on delivering individualized, patient-centered care in a home setting to support recovery, prevent decline, and enhance quality of life. Key Responsibilities Perform in-home physical therapy evaluations and functional assessments Develop and implement individualized treatment plans based on patient needs and home environment Provide skilled interventions including therapeutic exercise, gait training, balance training, and functional mobility training Assess home safety risks and recommend adaptive equipment or environmental modifications Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs Monitor patient progress and adjust treatment plans as needed Document patient care accurately and in a timely manner according to agency and regulatory standards Collaborate with physicians, nurses, and other members of the healthcare team Travel to patient homes within assigned geographic area Qualifications Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Current state Physical Therapist licensure CPR certification (or ability to obtain) Valid driver’s license and reliable transportation Prior home health or outpatient experience preferred Skills & Abilities Strong clinical reasoning and ability to work independently Excellent communication and patient education skills Ability to adapt treatment approaches to varied home environments Strong organizational and time management skills Compassionate, patient-centered approach to care Work Environment Patient homes within assigned service area Regular travel required Flexible schedule based on caseload and patient needs Physical Requirements Ability to assist patients with transfers, ambulation, and mobility tasks Frequent standing, walking, bending, and light-to-moderate physical activity Ability to safely work in non-clinical environments Objective of the Role To improve patients’ independence, safety, and overall function by delivering high-quality physical therapy services directly in the home setting. #J-18808-Ljbffr University Home Care
